- Records concerning visits: 3 months (Protection of Communications Secrets Act)
4. Procedures and Methods for Destroying Personal Information
The Clinic destroys information immediately after the purposes of collecting and using it have been achieved. The procedures and methods are as follows.

Personal information stored in electronic files is deleted using technical methods that prevent recovery of the records.
Personal information printed on paper is destroyed by shredding or incineration.
5. Provision and Sharing of Personal Information (Only Where Applicable)
Except with your consent or as prescribed by applicable laws, under no circumstances does the Clinic
use your personal information or provide it to another person, company or institution beyond the purposes announced under "Purposes of Collecting and Using Personal Information."

- Where necessary for statistics or academic research, providing information processed so that a particular person cannot be identified

6. Outsourcing the Handling of Collected Personal Information (Only Where Applicable)
The Clinic outsources personal information as stated below to perform its Services. In accordance with applicable laws, outsourcing agreements
specify the matters necessary to ensure personal information is managed securely.

7. Rights of Users and Legal Representatives and How to Exercise Them
Registration of children under 14 (hereinafter "Children") uses a separate form with plain language that Children can understand.
The consent of a legal representative is always sought when personal information is collected.
To obtain a legal representative's consent, the Clinic collects minimum information from the Child, including the representative's name and contact details,
and obtains consent using the methods set out in this Privacy Policy.
A Child's legal representative may request access to, correction or deletion of the Child's personal information.
To access, correct or delete a Child's information, click Edit Member Information and complete verification of the legal representative.
The legal representative may then directly access, correct or delete it, or contact the privacy officer in writing, by telephone or fax for necessary action.
The Clinic does not provide or share information about Children with third parties. Where a legal representative requests correction of an error in information collected from a Child,
the Clinic prohibits use or provision of that personal information until the error has been corrected.

8. How to Withdraw Consent / Membership
You may withdraw at any time the consent you gave at registration to the collection, use and provision of personal information.
To withdraw membership, click "Withdraw Membership" on My Page on the Clinic's website and verify your identity, then withdraw directly.
Alternatively, contact the privacy officer in writing, by telephone or fax; the Clinic will take necessary action, including destroying your personal information without delay.
9. Installation and Operation of Automatic Personal Information Collection Devices and How to Reject Them
The Clinic uses cookies to store and retrieve your information from time to time.

and stores on your computer's hard disk.

11. Measures to Ensure the Security of Personal Information
The Clinic implements multiple security measures as technical safeguards for Users' personal information.
All information you send is securely retained and managed in a security system protected by a firewall.
As administrative safeguards for Users' personal information, the Clinic has established procedures
for accessing and managing personal information, limits the number of staff handling it to a minimum, and provides continuous security training.
The Clinic also designates users of systems processing personal information, assigns passwords and renews them regularly.
12. Duty to Announce Policy Changes
This Privacy Policy was established on September 13, 2013. If laws, policies or security technology change,
and content is added, deleted or amended, the Clinic will announce the reasons and details on its website
at least 7 days before the amended Privacy Policy takes effect.
